Make a Referral
One-Eighty takes its responsibility regarding the protection of personal privacy very seriously. All personal data will be held and processed in accordance with the requirements of the 1998 Data Protection Act.
Fill out ALL the fields – all completed referrals will receive a response within 48 hours of receipt. Referrals will be allocated to workers using the Traffic Light safe guarding framework below. Families must be notified and must give verbal consent for a referral (and information sharing). Once the referral ha been accepted they will be required to give written consent.
The Traffic Light System
RED (high level vulnerability)
Criteria
- Young Person/ Family actively involved with other professional agencies (i.e. social care/YOS)
- Young Person is having difficulties in both school and their community.
- Previous family issues continue to affect the Young Person or siblings
What the Young Person/school will receive
- A full assessment of the young person's needs consulting with both school and family
- A thorough one-to-one mentoring programme based in school with a skilled One-Eighty staff member- who can also act as Lead Professional - for a minimum of 3 months.
- Whole family support (if necessary and appropriate)
- Nominal hourly cost of £15 per hour charged to the referral agent.
AMBER (medium level vulnerability)
Criteria
- Young Person has had long term behaviour difficulties
- Young Person has difficulties with peer to peer relationships and/or bullies others
- Young Person has previously had involvement from Social Care or other professional agencies but these are no longer involved.
What the Young Person/school will receive
- A structured one-to-one mentoring programme more focused on their behaviour management
- A mentor currently in training in a relevant profession i.e. is a psychology/social care student
- The mentor may have minor contact with the young person's family to work through relationships between home life and school behaviour
- A free, volunteer-lead service
GREEN (low level vulnerability)
Criteria
- Young people who struggle to engage in mainstream classes due to low self-esteem, lack of confidence, or who find it difficult to interact with other young people successfully
- Young People who have few positive role models
- School have noticed an unusual change in behaviour or a change in behaviour related to an event (i.e. a bereavement)
What the Young Person/ school can receive
- A mentoring programme with informal one-to-one sessions mainly outside of school time, with the opportunity to develop activity groups with young people in a similar situations i.e. art groups, game nights, sports activities.
- A free, volunteer-lead service
